WHY IS THIS ASSESSMENT REQUIRED
Before a clinician can assess for attention-related conditions, it is important to first screen for other factors that may affect concentration, mood, behaviour, or functioning.
Many conditions can produce symptoms that look similar to attention difficulties, including anxiety, depression, sleep disorders, stress-related conditions, substance effects/use disorders, and medical factors.
This structured screening questionnaire helps your clinician identify whether any of these factors may be contributing to your symptoms.
Completing this step ensures that your assessment pathway is safe, appropriate, and clinically thorough.
This screening does not assess for ADHD and does not provide a diagnosis.
It is an initial information-gathering step used to guide your clinician.
WHAT TO EXPECT
The assessment
Takes about 30–45 minutes
Can be completed on phone, tablet, or computer
You can pause and return if needed
Your responses are confidential and securely stored
Only your referring clinician can access your results

CogniBox does not provide diagnoses or treatment decisions — your clinician will review your results and discuss them with you during your appointment.
No personal details are stored, only the unique User ID that your clinician provided you with.
What Next
Once you have successfully completed your CogniBox screening and ADHD questionnaires,
Your responses will be securely processed and a structured report has been generated for your doctor.
This report summarises your answers and highlights clinically relevant information to support their review.
