These Terms & Conditions govern access to and use of the CogniBox platform (“CogniBox”, “we”, “our”, “the Platform”). By accessing or using the Platform, you agree to comply with these Terms.

If you do not agree, you must not use the Platform.

2. Nature of Service

CogniBox is a structured digital assessment support platform designed to assist clinicians in preparing for ADHD and related mental health assessments.

CogniBox:

  • collects patient-reported responses via digital questionnaires

  • applies automated scoring logic

  • generates structured summary reports

  • highlights potential symptom domains and screening indicators

CogniBox does not:

  • provide medical advice

  • diagnose medical or psychiatric conditions

  • prescribe medication

  • replace clinical assessment

3. Medical Disclaimer

CogniBox is not a healthcare provider.

All information generated by the Platform:

  • is based solely on patient self-report

  • is screening information only

  • is not diagnostic

  • must be interpreted by a qualified healthcare professional

Clinical decisions, including diagnosis, prescribing, and management, remain solely the responsibility of the treating clinician.

If you are experiencing a medical or mental health emergency, contact emergency services immediately.

4. Eligibility

You may use CogniBox only if:

  • You are aged 18 years or older; or

  • You are using the Platform under the supervision of a qualified clinician or guardian.

You must not use the Platform for unlawful purposes.

5. User Responsibilities

Patients agree to:

  • Provide accurate and honest responses

  • Complete assessments personally unless instructed otherwise

  • Seek clarification from their clinician regarding results

Clinicians agree to:

  • Independently assess and verify clinical information

  • Not rely solely on automated outputs

  • Accept full responsibility for clinical decisions

6. Data Security & System Limitations

We implement reasonable technical safeguards, including encrypted transmission and secure data handling practices.

However:

  • No online system is entirely risk-free

  • Transmission over the internet carries inherent security risks

  • We cannot guarantee uninterrupted availability

7. Intellectual Property

All content on CogniBox, including:

  • scoring logic

  • layout

  • system design

  • reports

  • assessment formatting

is the property of CogniBox or its licensors.

Unauthorised reproduction, distribution, or modification is prohibited.

8. Limitation of Liability

To the maximum extent permitted by law, CogniBox is not liable for:

  • clinical decisions made by users

  • misinterpretation of screening results

  • inaccurate self-report data

  • system downtime

  • indirect or consequential losses

Use of the Platform is at your own risk.

9. Indemnity

Users agree to indemnify and hold harmless CogniBox from claims, damages, or liabilities arising from:

  • misuse of the Platform

  • breach of these Terms

  • unauthorised access attempts

10. Fees

Where fees apply, they will be disclosed prior to use. Fees are non-refundable unless required by law.

11. Termination

We reserve the right to suspend or terminate access if:

  • Terms are breached

  • misuse is detected

  • security concerns arise

12. Changes to Terms

We may update these Terms periodically. Continued use constitutes acceptance of updated Terms.

13. Governing Law

These Terms are governed by the laws of Australia. Any disputes shall be subject to Australian jurisdiction.

CogniBox is a structured digital assessment support system designed to assist healthcare practitioners by collecting and organising patient-reported questionnaire information prior to clinical consultation.

CogniBox is not a medical service provider and does not offer medical care, clinical diagnosis, treatment, therapeutic intervention, or prescribing services. The platform functions solely as an information-gathering and reporting tool.

2. No Medical Advice

Information presented within CogniBox, including questionnaire items, automated scores, summaries, flags, or generated reports:

  • does not constitute medical advice

  • is not intended to diagnose any condition

  • must not be relied upon as a substitute for professional clinical judgement

  • should not be used to make health decisions independently

All health-related decisions must be made in consultation with a qualified healthcare professional.

3. Screening Tools Only

All questionnaires used within CogniBox are screening instruments. Screening tools:

  • identify patterns or symptom indicators

  • do not establish diagnoses

  • do not confirm or exclude medical conditions

  • may produce false positives or false negatives

Final interpretation must always be performed by a licensed clinician who considers clinical history, examination findings, collateral information, and professional judgement.

4. Self-Reported Information

CogniBox relies entirely on information entered by users. The platform:

  • cannot verify accuracy of responses

  • cannot confirm identity of respondent

  • cannot assess context or nuance

  • cannot detect intentional or unintentional misreporting

Results may be affected by incomplete, inaccurate, or misunderstood responses.

5. Clinical Responsibility

Use of CogniBox does not transfer or share clinical responsibility. The treating clinician retains sole responsibility for:

  • diagnosis

  • prescribing decisions

  • treatment planning

  • risk assessment

  • follow-up care

CogniBox does not assume responsibility for clinical outcomes or decisions made by healthcare practitioners.

6. No Therapeutic Relationship

Use of the CogniBox platform does not create:

  • a doctor–patient relationship

  • a therapeutic relationship

  • a duty of care

Any therapeutic relationship exists only between the patient and their treating clinician.

7. Risk and Safety Limitation

CogniBox is not an emergency or monitoring service.

The platform:

  • does not monitor responses in real time

  • does not provide alerts to emergency services

  • does not provide crisis intervention

  • cannot respond to urgent mental health risk

If you are experiencing distress, thoughts of self-harm, or a medical emergency, you must immediately contact emergency services or an appropriate crisis support provider.

8. Platform Limitations

You acknowledge that:

  • automated scoring is algorithm-based

  • summaries are computer-generated

  • technical errors may occur

  • system interruptions may occur

CogniBox does not guarantee uninterrupted operation or error-free performance.

9. Appropriate Use

CogniBox is intended solely for use:

  • as a structured assessment support tool

  • under referral from a clinician

  • as part of a broader clinical evaluation

It must not be used:

  • for self-diagnosis

  • for independent clinical decision-making

  • as a replacement for medical care

10. Acceptance of Disclaimer

By using CogniBox, you acknowledge and agree that:

You understand the platform is not a medical service
You understand results are screening information only
You will seek professional medical advice for health concerns
You will not rely solely on CogniBox results for medical decisions

If you do not agree with this disclaimer, you must not use the platform.
